Membership Options and Costs. The Member may select one of the following options:
Premium Membership for an Adult
$449/year
If you have elected to join at the Premium Membership Level, you will receive:
- Two basic cleanings per calendar year (prophylaxis)
- One full mouth X-ray on the first exam of each calendar year.
- Required X-rays for exams and cleaning appointments (Bitewings 2, PA 2).
- Oral cancer screening.
- Oral hygiene instruction.
- Fluoride treatment twice a year.
- One emergency visit, which will include examination only. Any treatment required at the emergency visit shall require additional payment.
- 15% discount on all other dental services.
- Free whitening tray if clinically appropriate, as determined by Provider in its sole discretion.
Standard Membership for an Adult
$349/year
If you have elected to join at the Standard Membership Level, you will receive:
- Two basic cleanings per calendar year.
- One full mouth X-ray on the first exam.
- Required X-rays for exams and cleaning appointments.
- Oral cancer screening.
- Oral hygiene instruction.
- One emergency visit, which will include examination only. Any treatment required at the emergency visit shall require additional payment.
- 15% discount on all other dental services.
Membership for a Child (0-13 years)
$299/year
If you have enrolled a child less than 13 years old (at the time of enrollment) as a member, you will receive:
- Two basic cleanings per calendar year.
- One full mouth X-ray on the first exam.
- Required X-rays for exams and cleaning appointments.
- Oral cancer screening.
- Oral hygiene instruction.
- One emergency visit, which will include examination only. Any treatment required at the emergency visit shall require additional payment.
- 20% discount on all other dental services.
- Fluoride treatment twice a year.
Terms and Conditions. This Agreement and the membership created hereunder is subject to the following terms and conditions: Membership is valid for 12 months from the purchase date. Any benefits not used by the expiration date shall be null and void. Membership is only valid at either Avondale Family Dental Care or AAA Dental Care, as selected by the Member at the time of purchase. The membership cannot be transferred from one location to the other. Exams and cleanings are redeemable no more than once every 6 months, including fluoride treatments. Either party may cancel this Agreement at any time upon thirty (30) days’ notice to the other party. Provider may cancel this agreement at any time for cause including, but not limited to, Member non-compliance with instructions and Member’s breach of any of the terms hereof. Upfront payment is required and no refunds will be provided. If either party cancels the membership prior to the expiration date, any prorated balance will be credited to the Member’s patient account. Can only be purchased with cash or credit/debit card. The membership services are non-transferrable and can only be used by the Member named herein. Provider’s cancellation and no-show policies will apply to any appointments set by Member. By signing below, the Member is affirming that the Member does not have dental insurance. In the event Member obtains dental insurance coverage, Member shall promptly notify Provider. Each appointment can only redeem one exam, X-ray, cleaning, and fluoride treatment, and Member must wait at least six months before redeeming any additional exams, X-rays, cleaning, and fluoride treatments. The six month waiting period shall not apply to emergency treatment.
